New Client Horse Details Form Template

Start every horse relationship properly

When a new horse arrives on the yard, accurate information protects your team, your clients and the horse.

This template allows you to collect:

• Horse identification and passport details

• Vaccination status and medical history

How to Use & Edit the Google Form Templates

We’ve uploaded ready-made Google Form templates in the members’ area to help you get started quickly.

Step 1: Open the Template

• Go to the Members’ Area → Templates / Resources
• Click on the Google Form you want to use

Step 2: Click the Blue Button

• When the form opens, click the blue “Use this template” button
• This will automatically create your own editable copy in your Google Drive

⚠️ You must do this step: you cannot edit the original version.

Step 3: Rename Your Form

• At the top left, click on the form title
• Rename it (e.g. “Booking Form – Your Business Name”)

Step 4: Edit Questions

You can fully customise the form:

• Click on any question to edit the text
• Use the dropdown on each question to change the type (e.g. multiple choice, text, checkbox)
• Tick “Required” if the question must be answered
• To add a new question, click the + (plus icon) on the right-hand side
• To duplicate a question, use the duplicate icon
• To delete a question, click the bin icon

Step 5: Update Branding

• Click the paint palette icon (top right)
• Add your logo/image
• Change colours to match your brand
• Choose different font styles if you wish

Step 6: Adjust Settings

Click the Settings tab and review:

• Collect email addresses (optional)
• Limit to one response per person (optional)
• Edit the confirmation message after submission

Step 7: Preview Your Form

• Click the eye icon (Preview) to see how your form will look to clients
• Test it before sending

Step 8: Share Your Form

• Click Send (top right)
• Choose how to share:
  • Copy the link
  • Email directly
  • Embed on your website

Step 9: View Responses

• Click the Responses tab
• View answers in the form
• Or click the green Sheets icon to create a spreadsheet

Troubleshooting

If you can’t edit anything:

• Make sure you clicked “Use this template” (not just viewing it)
• Check you are logged into your Google account
• Try refreshing the page or opening the link again
